Quick answer
Track each workflow's trust stage, review rate, decision rights, evidence, and next graduation gate instead of treating trust as one general rating.
Complete one row for every repeated workflow the VA owns. Review the matrix monthly during the first six months and quarterly after the role stabilizes.
| Workflow | Risk if wrong | Current stage | Review rate | Decision rights | Evidence | Next gate | Review date |
|---|---|---|---|---|---|---|---|
| ______ | Low / medium / high | Instruction / execution / correction / consistency / autonomy / trust | ______ | Decide / recommend / never | ______ | ______ | ______ |
| ______ | Low / medium / high | ______ | ______ | ______ | ______ | ______ | ______ |

Questions for the monthly review
- Which workflow has enough evidence to move forward?
- Which workflow still creates avoidable questions or corrections?
- Are exceptions being raised at the right time?
- Does the procedure cover the errors found this month?
- Which decision right can the practice widen safely?
- How many manager hours did supervision require this month?
When a workflow advances, write down what changes. For example: “Routine benefit verification moves to 20 percent random review. The VA may resolve standard payer discrepancies and must escalate plan changes or conflicting information.”
