Before access
Complete the privacy, security, training, device, connection, and workspace requirements that apply to the role. Record the evidence and responsible person.
Prepare an access matrix with the system, permission level, approver, provisioner, date granted, and review date.
Stage 1: Communication and documentation
Create the practice-issued email, approved team channel, calendar, and documentation access. Do not use personal accounts.
Stage 2: Read-only operational access
After training sign-off, grant only the read access needed to observe and practice the workflow. A recall role does not need the ledger. A reporting role does not automatically need clinical notes.
Stage 3: Role-scoped write access
After guided work has been reviewed, add the exact write permissions needed for the role. Use named accounts and multi-factor authentication.
Stage 4: Additional systems
Add other systems only when the workflow requires them and competence has been demonstrated. Avoid access granted “just in case.”
Day-30 review
Check every account and permission. Remove anything unused, correct excess access, and record the review.
Offboarding
Maintain the revocation list from the first day. If the relationship ends, disable access the same day, recover or wipe managed equipment, confirm data return or deletion, and record completion.
