Direct cost
Record the wage, hourly rate, monthly fee, percentage, platform charge, employer-of-record fee, equipment, licenses, and any setup or conversion fee.
Recruitment
Count the time spent writing the role, sourcing, screening, interviewing, testing, checking references, and negotiating. Value the owner’s and manager’s time at a realistic internal cost.
Onboarding
Include process documentation, account setup, equipment, privacy and security training, shadowing, feedback, and early quality review. A provider may cover some of this, but the practice still has to teach its own rules and workflows.
Ongoing management
Estimate check-ins, work review, exception handling, performance management, leave planning, and process updates. Confirm whether a provider manages the person’s employment, daily output, or both.
Continuity
Ask how holidays, sickness, turnover, and sudden departure are handled. Price the disruption if one person leaves and the practice must start again.
Cost of failure
Estimate a realistic failed-hire scenario:
- Recruitment repeated
- Management time already spent
- Work delayed or redone
- Access removed and reprovisioned
- Revenue or patient follow-up missed during the gap
- A new person trained from the beginning
Replacement coverage can make a higher-rate option cheaper than it first appears. A successful direct hire can become less expensive over several years. Both statements can be true.
Compare like with like
For each option, write:
Year-one direct cost + internal time + setup + management + expected continuity or replacement cost
Then compare what the practice receives. One quote may supply a person. Another may include recruiting, equipment, compliance support, management, and backup coverage. The rate alone cannot show the difference.
